SD Card Problem (Apacer 256mb)

SD Card problem with my XDA cannot format (error code 31) anyone know how to fix this, tried storage tools to format it but did not work pls help

Flash cards get brokes very often. Replace your SD card.

why does my sd cant be read by camera after formating it?

because after i format it in my computer, i try it to insert to my camera but the camera just say "NO SD CARD INSERTED"
