I downloaded the tools Alex posted in his thread, followed the directions, and after about 15 attempt finally ended up with a ROM for my ATT Tilt that I can live with. It is based off of his ultraclean ROM, but with additions I find tolerable. The start menu is very clean, with no shortcuts activated. When I first loaded Alex's ultra lean ROM with no shortcuts on the start menu, I was sure it would not be long before I filled it with shortcuts. I have since decided to leave it barren, since it fills itself with recently used programs, and this is far more useful to me. You can create the shortcuts yourself if you like.
This ROM does not have PTT in it, nor does it have any reference to it whatsoever (no context menus, etc.). The button is free to map as you wish. Good riddance to what I find to be a useless application. I did not include an MMS composer, opting to download one from the CAB collections all over this forum. The packages I included from the kitchen are:

This ROM includes radio 1.27.14.09
This ROM does not include any of the menu tap sounds.
This ROM only includes 1 ringtone (Ring-Standard). I do not like to clutter my device with ringtones I never use. If you want more ringtones, go here: http://www.megaupload.com/?d=Q039KPOU .
This is a collection of sounds I have saved over the years and keep on my storage card when I want an interesting sound or ringtone. It contains most (if not all) ringtones found on Pocket PC Phones in the last several years. Put whichever ones you want in Windows/Rings or in My Ringtones in your My Documents folder on your device.
For whatever the reason, when I cooked this ROM some of the ATT Tilt stuff gets installed (like the ATT Theme, Media Net, ATT Mall), but not all the rest of the bloatware, and the internet sharing tool is available as well.
I installed the new camera floating around (the stock camera is installed by default), as well as HTC album (needed for the new camera). I installed the new Touch Dual Dialer by Navadi and Dutty modified by Shayd found in this thread at post #9 (how is that for spoon feeding): http://forum.xda-developers.com/showthread.php?t=355085
This dialer includes the useless Video Dialer buttons and context menus. Nonetheless, it has big and easy to read and use buttons and it has the ability to reject a call with a text message. Try it out. If you do not like it, uninstall it or use KaiserTweak or Advanced Configuration utility to change it to the stock dialer.
I also installed a bunch of other CABS, but I am not going to get into that now. I only mention the new camera and HTC album because they work very well together. You can find them through this thread: http://forum.xda-developers.com/showthread.php?t=355587 .
This is a base working ATT Tilt ROM that you can customize as you want. It is quite fast and stable on my device.
There is no icon for Cellular Video. You can get to it by going to cingularvideo.cingular.com . Bookmark the site if you like. It will open up videos in the installed RealPlayer. You can watch videos from m.youtube.com, but, by default, they will try to open up in RealPlayer and fail. To fix this just go to the registry and navigate to HKEY_CLASSES_ROOT\rtsp\Shell\Open\Command and change RealPlayer.exe to StreamingPlayer.exe. After you do this both Cellular Video and m.youtube.com will work.
