Keypad Cab

Can some one please, for the life of me, tell me what keypad this is and where to download it?
Please don't say it's the HTC Dual Touch, because that's the qwerty keypad that I am NOT looking for...

This to me looks like intellidialler's PhonePad that has been skinned.

I have .cab but not that skin.

I get the warning of untrusted certificate thing when I try to install. Any ideas?
