Since SpeakerPhone - Auto On didn't work for me, I wrote a short script to turn on the speaker when a call is in progress (you can turn it off by pressing the SpeakerOff key).
It weights only 12 KB (the script less then 1 KB)
Just put both file under \program files\scripts\
Make a shortcut for SPon and put it under \windows\startup
No need to restart, just run SPon.mscr

If it is not working for you find the value of "HKLM\System\State\Phone\Status" when call is in progress, and modify my SPon.mscr accordingly.
