I'm pretty keen to update to one of the newer ROMs (I'm using one of Dutty's from way back in March or so)
The problem is that I need to keep my SMS (and ideally my MMS) which are threaded. As I understand it PIM Backup 2.8 doesn't do threaded SMS and MMS - is there an alternative somebody can suggest?

It works fine for me, I have used pim 2.8 with threaded sms/mms for a long time, and have no problems with it

I had no problem with PIM 2.8 and backup of SMS messages and textual part of MMS messages, from Threaded SMS app, but I have lost the pictures and sounds from MMS messages.

This question should have been posted in the kaiser general forum not the rom development forum. I know that you probably posted it here because it gets the most attention, but believe me when i tell you that most of the people who have the answers check the software and general forum as well. Having a lot of threads like this posted here just clutters this forum and makes it more difficult to keep up to date on the roms that are being developed. In addition, it is XDA policy to post in the relevant thread, and in the correct forum.
To answer your question hoever, I use pimbackup and i haven't had any troubles with it not restoring my attachments to mms messages.
