Need help with returning my polaris to original ROM

hello everyone,

I have some hardware issues on my polaris, and I want to send it to HTC.
I have udk7 ROM on my phone.
I had:
rom:
1.25.405.1 WWE

radio:
1.58.21.23

Protocol ver:
25.65.30.04H

what should I do that HTC will accept my polaris with the warranty?
I know I need to return the original ROM, but what about the HardSPL I installed???

I want to add and say that I downloaded
RUU_Polaris_HTC_WWE_1.25.405.1_radio_sign_25.65.30 .04_1.58.21.23_Ship
so, if someone could give me a procedure what shoud I install first and how to remove Hard-SPL (if necessary at all...)

Here you can find the original SPL and how to put it back to original.

If you only flashed the OS rom from UDK. Than you only need to install the RUU_Polaris_HTC_WWE_1.25.405.1.
