Damn! Whilst making calls my phone get's pretty hot guys - I just hung up from a pretty long conversation and the little Diamond was toasty!

I know from other threads that this can happen, but how can you distinguish whether it's a serious problem or not?

My Diamond only seems to get hot while charging, and when using CPU intensive applications, not really during phone calls. But apart from that, I don't really think you have a serious problem, as I'm sure the folks at HTC have tested every aspect of battery while in use.

mine gets also pretty hot if phonecall lasts longer than 45 minutes

Yeah, join the DEFC (Diamond Eggs Frying Club).

In the manual it notes that the phone may become warm with prolonged use. I've had mine for about 3 weeks and it doesn't seem to cause any problems. However, I'm starting to suspect that the phone warming is what causes the battery meter to wig out. When it gets too warm, the percentage no longer drops. Then all of a sudden I'll lose 20% battery in 2 minutes when the phone has cooled back down.
I downloaded PowerGuard and have been watching the battery % in relation to temperature and I'm pretty sure there's a coorelation. If not, then this is the worst battery meter I've ever had on a phone. Basically, it's just a rough ballpark guess. Couple this with a crap battery to start, and it gets pretty frustrating...
