Restoring back to regular

I need to restore my dash to regular om order to send it back to T-mobile. The Kabana made my top screen fuzzy, so I request a new one, since im still covered under warranty. Where do I start?

Not heard of the coco kabana lol, i think it's a custom ROM comes in yellow. :D
