I'm pretty sure that is Phone Alarm by PocketMax (http://www.pocketmax.net/phoneAlarm.html)

This program allows you to create "profiles" such as Home, Office, Car, etc which can change how your device works (e.g. ring or vibrate on/off, repeating alarms for SMS, etc). It comes with a Today screen plugin providing one-touch profile switching. This plugin can be "skinned" to look almost any way you like.

A few other developers have similar programs such as SBSH and SPB.
