I had the same problem. My camera didnt work for about 2 weeks. So I finally did a RMA and sure enough the day before I got my replacement it randomly started working again.

I did all the factory resets and unroot/reroot nothing could fix it. But one day my niece clocked the camera app and it all the sudden worked.

It looks like it was the kernel for me - Downloaded the latest Franco kernel from http://forum.xda-developers.com/show....php?t=1867127 (never heard of it before!) and now the camera works perfectly.

Experienced this issue twice, a reboot fixed it.
Nexus 5 32GB D820
Stock ROM, Franco Kernel

It looks like it's a 3rd party launcher issue. I froze all launchers except google now launcher. Camera worked for 3 days without any issue. Then I froze every other launcher except Action launcher. Issue popped up the next morning.

It seems to happen most often when the camera app is already started and in memory. If I start it, put the phone to sleep, wake it, then try to start it again from the lock screen it will usually crash. If I swipe it away in the recent apps list before putting it to sleep it will always start fine.

Switching to ART seems to have solved it for me. Had to give up xposed framework though

Just made an app to help with this problem, search "CMFix" in the Play Store, hopefully it helps

Honestly I don't know, I've not got any devices with stock ROMs on to try it with.

Worth a go though! Maybe someone could give it a try and report back?
