Nexus 5 dialer problem

I am facing a problem with my Nexus 5 dialer. Sometimes when there is an incoming call my screen goes blank and I am not to either see or answer the call. This problem stays until I restart the phone. After 1-2 days the problem starts again. Also I am getting 'proximity sensor' wakelocks while making calls. The wakelock stays for almost 80-90% of the call time. Anyone else is facing this issue? Is there a solution for this? I tried clearing the data and cache of the dialer app.

try disabling Non-Intrusive Call from Phone or from GravityBox (if installed)

I tried and it didn't helped. Was using Bluetooth for couple of days and the wakelock never came back. This wakelock is happening only when I'm not using a Bluetooth headset.
