Try clearing the cache and dirty flashing the ROM.

Yes I do press back quickly. U suggest slowing down? Lol. I think by selecting 'Don't keep activities' in developer mode stops it but it takes a little longer closing the app.

Looks like I will just have to live with it. No big issue, just being picky.

Actually Later today, I will do a fresh firmware flash and see what happens.

I was just thinking if pressing too fast you might be hitting something else. Computers are funny like that.

I was having this issue on my nexus 7. One thing I noticed was that it happened most often in landscape mode. Then I noticed that the navigation ring feature of the ROM had the default action trigger placed right under the back button. I think how the nav ring is getting triggered by back button presses. For me, my custom ROM allowed adding slots, which moved the position of the action. Turning the ring off entirely might also be an option if you don't use it.
