SD Card issue...

Recently I have been getting notifications that my SD Card was unexpectedly removed, then it will initialize and be fine for a while. I popped it in a reader and it seems to work ok with the PC. I reformatted the card and tried again with the same results. How can I check if it's the SD Card failing or a hardware issue with my Note 4? I don't have an extra card laying around to drop in the phone to try that unfortunately.
