I've unlocked, installed recovery, then perm root. Then it boots into recovery, and I get device not found when I try to do it. Every. Bloody. Time.

Sometimes when using the toolkit the phone doesn't get rooted. To get perm root after unlocking the bootloader and installing CWM copy this file to your phone and install it with Clockwork, then you should have permanent root.
