Hey mike,
The icon pack comes from here (u have to install Go Laucher first in order to get this icon pack, mm actually its a theme)
The Calendar (and also the FB + the sms) widget is from Go Launcher widgets its too easy to find them in market.
The icons for the folders (gamepad etc) i think its from here

I HAVE NO idea how to take a screenshot with CM9 but anyways this is how it kinda looks, but then im using LP PRO+ And HD Weather Widget

i press the power button, and under the tabs, restart, shut down, there is a tab saying "screeshot", after that, in the internal sd there is a folder called "pictures" (*)
(*) in order to copy them to pc, i have to restart the phone otherwise i see 0kb .png files
