Same card also. Tried a few things with no luck. It works fine with Recovery v5.

What exactly have you tried?
How do you format the Card?
Have you taken a cardreader and set the clustersize to default and the Filesystem to FAT32?

I will try that, it's a brand new card though.

Don't know if you use a cardreader or just put the phone on PC, but please try a cardreader and a clustersize under 4 KB f.e. 2048 Byte.

Put in s2 which running ics, format sd card, once completed, your card will be formatted to fat32. But when s2 running samsung jb, it will format your card to exFAT. After formatted to fat32, all the problems solved. Used pc to format this 64gb card to fat32, you need a special formatter, else it will ended became a 32gb card.
Cwm recovery and mobile odin not supporting exFAT. FAT32 more compatible to program in our phone but you lose out the speed offer by exFAT. exFAT when using in my s2 in usb mode, it read/write was fast. Eg when usb to pc, I can get 10mb/s under exFAT but only 4mb/s on fat32.
