Hi, this is what's happening to my phone:

Its Samsung Galaxy S4 I9500
I've used stock rom over a 3-4 months and install CRASH rom
and also CRASH rom worked also over 3-4 months and one day i got
CAMERA FAILED, i have read all kinds of threats about this problem
I can fix it when i do factory reset but after few days it happened again
so i install official rom without any root.. and it work for 2 weeks and it happened
again CAMERA FAILED, so is anybody have any advice what should I do
it's really frustrating, can't do factory reset all the time

Check if it is a rom problem or software problem:
Try to install official rom and do not download any software within 2 weeks.
If the camera failed again, then it is the rom problem.
If the camera works well during 2 weeks, then problem you said was caused by the software.

That is incorrect advice. It is either a software issue OR a hardware failure. A software issue might get fixed if you flash back to stock...but if its a hardware issue, it will persist irrespective of whether you flash stock firmware OR a custom ROM.

If it is a hardware problem, it can't work for 2 weeks after installing rom. So I don't think it is a hardware problem.

Here is an update :D
after installing and using stock rom it happened again after 2,3 weeks
and i'm using now 4.4.2 crash rom, and its working, over a month will see what's happening later...

Camera Failed error is more often than not a Hardware problem. I've faced this on my first Samsung - the GT-I9000, and no amount of Rom flashing could make it go away.

If you have message "Camera failed"...
First of all try this: Open Camera app from the "Application manager" and do "Clear data", then restart the phone.
Second: Do a Factory reset (of course save your data!).
Third: Reinstall Android OS.
If this not help you, you have a hardware problem. You need to change the camera.
