No.

Details? Did you format system/data/boot? Are you running a theme? Kernel?

Yes full wipe, + data and dalvik. I am running air kernel 4.0 original

I read on a few other forums that fixing permissions may help? Any ideas?

I say re download, wipe data cache and dalvik.. but make sure you format boot and system in the mounts and storage section of clockwork. after that, flash gapps, wipe cache and dalvik and then flash kernel.

I found that doing a battery pull causes the issue. I have a second battery that i swap out. Com.phone.android. Someone try a battery pull and see if u have the same issue.
Could it be the new 4-22 gapps?

Hands down M5 is for me the best rom around. Absolutely AMAZING and FUNCTIONAL. Not many roms out there are just ready to go completely right out of the gate without some fixes.

So far ZERO issues. Everything works perfectly.

Thanks guys

Taking a rooted phone into a sprint store is a no no. They can refuse warranty work. If you have a hardware issue, you need to return to stock or run the risk of a voided warranty.

Actually i read somewhere not sure where though that sprint has adopted the policy of taking rooted devices.

    *edit* I found it:
Don't know if I should be posting this or not, but some people still think it "voids the warrenty"

    Overview
Rooting a phone (or getting root) is the process of modifying the operating system on a
device to obtain complete control over it. By rooting a device, the user can then alter certain
built-in functionality of the device. Rooting is strongly discouraged by Sprint and device
    manufacturers.
Since there is currently no way of proving that a device has been rooted by a customer,
suspected rooted devices should be handled as follows:
    Advanced Exchange Locations
If the customer is experiencing issues that are suspect to be caused by rooting the device
    Step Action
    1
Educate the customer on the risks involved in rooting their device:
    • Rooting can cause irreparable damage to the device.
    • Rooting can result in an inoperable device.
    • Rooting may result in all customer data and customizations being lost.
    2
Open and eTicket and exchange the device per standard Advanced Exchange
procedures. NOTE: Refer to AE Transactions M&P for more information.
    3
In the eTicket, document the specific issues with the device and note that the device is
    suspected of being rooted.
Full Service or Service Only Locations
If the customer is experiencing issues that are suspect to be caused by rooting the device
    Step Action
    1
Educate the customer on the risks involved in rooting their device:
    • Rooting can cause irreparable damage to the device.
    • Rooting can result in an inoperable device.
    • Rooting may result in all customer data and customizations being lost.
    2
Open an eTicket and document the specific issues with the device. Also, note that the
device is suspected of being rooted.
3 Attempt to resolve the issue by reloading the device software.
    4
If the issue cannot be resolved, exchange the device per standard Advanced Exchange
procedures. NOTE: Refer to AE Transactions M&P for more information.

    Hide the nav bar.

Try this to hide the nav bar:
ROM control, power menu options, enable navigation bar (enables nav bar toggle in power menu..)
Then hold power button, disable (or enable) nav bar.
    Reboot somewhere in between.

With power menu nav option enabled, holding power, then press toggle below.
