After I restart my phone, my home screen will shift icons around. Some will be missing, random widgets will be present that I never put there, the content of screen 2 will shift to screen 1, etc. This started happening after I performed a factory reset. I'm using stock jb on my RAZR maxx. Please help!

It could be the popular culprit back in the old time called emmc data corruption bug. I recall there is some application on the market to check for emmc integrity.
