I also use a Rom: ICS 2.2.1! everything works fine!
Try to wipe dalvik cache!

decompiled your and original files from ICS 2.2.1, and did not find any changes. ???? lol

it didnt work on my phone either , i have sense 3.6 , android version 4.0.3 , insanly cool sense V2.2.1. what do you think the reason.
i installed teh zip from recovery.

try to manually replace the file in a folder system, using root explorer! and then put the correct permissions! reboot

Still not working i made install from recovery and recopy too. If you recopy this the clock disapiring.

gabbana-music thanks man for your work

i rly love it.

You say its working for you on ICS 2.2.1 rom. Did you install your Full transparency to the bottom bar of the sense 3.6 + [MOD][Transparent StatusBar][ICS] for Multiple ROMS! or you test it on clean install? Cuz with this mod your Clock mod is not working. I was tested on my 3 friends phones and they had the same problem. ;/

I use Insanely Cool Sense 2.2.1, with these two mods! just that, put the original htc widget clock, and then again modified, and everything works fine!without even wipe dalvik cache! I do not know why you do not get to do it!
