Rom: cm7
Launcher: adw ex
Apps: Minimalistic text for the battery meter, mClock for the clock and Beautiful widgets for the weather. The icons i cant remember who made them but i tip is to visit android themes here on xda tons of stuff in there. Hope it helps

That's clockr, if I'm not mistaken. I use the same app on my device. You can find a free and a paid version in the market.
