Could be a number of issues but main cause is normally a corrupt file on sdcard or having your sdcard read ahead speed set too high.

It happened to me once
Backup the contents of ur card to ur computer then format SD card
while u are copying u may see some errors because there will be some corrupted files (Which can't be deleted) just skip the copying of those files, they'll be deleted while formatting

When you format ,you can still experience the issue and if that happens than you have a damaged sd card ,this same problem came up on my old wildfire with a bad sd card from ebay .

??? This doesn't mean you have a bad sdcard at all. I had this issue many times after mounting my phone to PC. I fixed by lowering the read ahead speed / eliminating a corrupt file on sdcard ( this could be any file, run a disk check whilst mounted to PC) I also only mount from recovery now and have not had the issue since.

Perhaps, but in my case the recovery didnt read it ,neither did the device, formatting on pc unsuccesful - Dead SD.

And yes ,before the critical end when I got the read-only issues, even if I did format for 4 times after each time that error showed up.
