That looks bad. Looks like it has something to do with the change I put in for formatting the system.
I suggest you restore your system first and not try again. Probably the command I put in is not compatible with US kernel. I hope it doesn't brick your device.
EDIT: Just double check. Are you using US or Intl version?
EDIT: When I look at your previous log again, it seems that your /etc/fstab is missing from your system which result in a failed mount and the install went horribly wrong. However, in previous successful install, the /system directory is probably mounted already before you start flashing and a failed mount doesn't matter. In the latest patch, there are changes that will unmount /system first but the script fails to mount it again because of the missing file. I think it is important to check whether you are using US or Intl kernel and whether the missing file is normal or not.
