Thanks for the reply. But it's without anything synced. Proof = doesn't happen with 2G. Once it's -43mA, it's constant until I go back and forth 2G/3G. So "peaks" a bit misleading.
Could it be some kind of communication between network operator and mobile that hangs or goes into loop? Since it's only with 3G, maybe it's something my operators 3G cell asks my phone..

You can really only test battery drain with airplane mode on as so many things could be happening when your data is connected, like I had 60ma battery drain during sleep with only airplane mode, I narrowed it down to Google service framework keeping a wake lock, so if toggling 2g solves it, it's most certainly an app or something in the background keeping a wake lock

Download better battery stats. Or look in spare parts at battery history, or download battery monitor widget
