It's a bug on Samsung wp7 phones(some said) and tango is a beta ROM(it's not released to the public so it still has some bugs). Run samsung network profile app(marketplace Samsung apps) select a different network carrier as yours. wait to set it (you will see 3G/E sysmbol) then select your network carrier(it depends on what network you are). If your on at&t try it (some people said it works). I'm on Straight Mobile and had this problem on the stock ROM and didn't work(this app only gives you 2 choices AT&T & ATT for my phone at least). Try flashing [ROM] Clean ROM - Stock METRO UI http://forum.xda-developers.com/show....php?t=1528984 ver. 1.30(mango) it works flawlessly. Tango is still buggy because it's a leaked ROM and not final.

No, the phone should be unlocked irrespective of the number of times you flash/format it. AFAIK, the unlock is in NVRAM aka non volatile RAM.
