FWIW,
You may also want to set the ownership on the "new" file to root / root. This will match the other files in the directory.
Today, I restored my original SystemUI.apk, which I extracted from a nandroid of my stock system.
1) Renamed it while on the SD card to SystemUI.bak, then copied it to /system/app with Root Explorer.
2) Set the permissions as Mr. V has shown. Set ownerhsip as I said 
3) Renamed SystemUI.apk to SystemUI.mod, then quickly renamed the .bak to .apk
EDIT: 1a) Set the file system to R/W first, then copy. 2) Dial 1-800-REBOOT
You can probably apply the mod this way too...
