This may be unrelated to your volume issue, but I did find out the hard way that silent mode only affects ringtones and notifications, not media - and some notifications from some apps seem to register as media, therefore ignoring the mute placed on notifications.
If that is the case, then the work around that has worked for me is to manually lower media volume - it is far from convenient, but it works. Also, the expandable volume menu option offers a much quicker way to get to the media volume slider, which helps a bit...
