You need root, and a custom rom like cm9/cm10 that supports themes(I'm using this rom), then download one of these themes from the play store. Click Here

Thanks, love it! I'm running helly bean nightlys. I got the cobalt theme how do I get the awesome icons that you have?

Go to settings/themes apply cobalt, apply anyway, after the theme installs I usually have to reboot to get the icons to appear.

I'm changing wallpapers daily, this one happened to be my favourite for today i use an app called Pattrn to change wallpapers daily
