It happens to me same thing if the file is not in sd card using an external app to flash.. could it be on ext sd or cache when downloaded from app?
The reason of the thing could be that the app is writing an incorrect path in the openrecoveryscript file to flash directly as the recovery start..
Only a supposition, of course

nothing shown in log
The new thing is some partitions being unmounted before installing zip files. The updater-script is responsible now on mounting them.
Can you post the updater-script contents of the zip?

Yes I know.
The zip only contains a theme.txt, a preview.jpg file and a framework-res folder with res subfolder with drawable-hdpi and drawable-xxhdpi folders with some image files.
So the updater script /logic must come from ROM custimzer app ... but I haven't found it yet.

seems like some shell script relying on some files in /cache/recovery
Those files are put there maybe by some other process/stage in the installer

Not possible to say without the whole zip installer + details to reproduce the issue
As per se, the shell script won't work as it doesn't mount any partition before starting to work on them
