You used OFFICIAL TWRP WITH hboot 2.16!! I would strongly advise you to switch to a TWRP thats built for 2.16 (like the one in the op), if not you could potentially HARD BRICK your phone

2.15 here. Trying to flash the image via the flash IMG GUI. App says success, booting into recovery I'm still 2.8.1.1.. Any opinions?

Use standard fastboot flash or other apps like twrp manager or rashr.

I had this problem too. If you want to install the recovery right from your phone, you can try flashify. This app works - at least for me.

\twrp manager only does zip. the dl for the recover is an .img.. flashify worked.. if flashify wouldnt have work i would have had to get out the laptop and do it the fastboot way.. was trying to minimize my laziness.. thanks.

TWRP manager flashes .img via "Install TWRP", .zip is for flash via "Install" and that's for ROMs/kernels/etc. Just pick up last from the list and it's latest version for sure. Doing that for a long time. Anyway, glad to see that you settled it
