I was facing the same problem like urs while rooting
But i came to know why it was happening
When i flash cwm 6.0.4.5 it just rebooted my phone so later i flash cwm 6.0.4.7 n problem was solved
So i advice u to download latest recoveries its available in development thread

Flash it using flashify. It can be found on play store. Worked for me.
