Why do some Android phone apps have a separate Tablet app?
It would seem more logical to have just 1 app that knows how to render/behave differently if it was running on a tablet device, than to have to manage 2 apps.

You bring up a very logical point, some tablet apps are extremely outdated from the days where some developers and in particularly companies thought it was just easier to separate the phone and tablet versions. Again it's all up to the developer and their preferences, personally when I develop I'd rather maintain one app then two. As Android tablets continue to evolve this problem will continue to diminish and get ironed out for the most part.
