Hope it works, it should. And don't reboot the phone while flashing/after erasing /formatting the bootloader!

I'm having the same problem as OP. What you are saying is to run the following, one command at a time, right?
./fastboot erase boot
./fastboot erase cache
./fastboot erase recovery
./fastboot erase system
./fastboot erase userdata
./fastboot flash bootloader bootloader-mako-makoz20i.img
./fastboot flash radio radio-mako-m9615a-cefwmazm-2.0.1700.84.img
./fastboot -w update image-occam-jwr66v.zip
