Apologies for your issues you are facing, and my response is probably not what you want to hear. However, why not install CyanogenMod 10.1.3 STABLE onto your EMMC/Internal storage, instead of onto an SD card? So, installing to EMMC instead of following the Hybrid/Pure SD installation may be a better method for you.
I'm running the stable version of CyanogenMod 10.1.3 (Android 4.2.2) on EMMC for both the Nook HD/+ and it works perfectly with no issues. I'm able to connect to and pair with other bluetooth devices no problem.
