Can't mount Ext SdCard

hello can someone help me? i can't mount my ext card it says 'safely removed' after booting....


this happened when i install cwm 6.0.3.5 for this device so i can use adb to unbrick my phone from chainfire3d installation....

can someone help me with this matter? i heard fix permission can help me mount the sd card... i'm using SanDisk 64gb... it's still running fine just tested it on my PC and my cousin's phone.. Thanks in Advance...
