PUK has nothing to do with SIMlock. PUK is a code you can use on the phone if the phone blocks because you tried a wrong PIN more than 3 times. That PUK code comes with the SIM card your phone provider gave you (with the phone itself when you buy phone device and phone service at the same time).
SIMlock is a lock on the phone that some phone providers include on the phone to prevent the phone from being used with a SIM card of other phone providers.
It is the phone provider the one that has to provide the SIM unlock code. I bet some providers will charge for the code.
