If I'm running Elegancia ICS Sense 4 Build 3.6 can I use the following theme:

Also, do I just flash it like a ROM? If not, how do I install it?

Yes, but remember to download the Sense 4.0 version of the theme. Make a backup, then flash like a ROM.
