There are some suggestions online that the "unable to get BOS descriptor" is due to udev rules not being set. If you are running the command as root anyway, there is no need to set udev rules.
I just wanted to rule that out before going any further.
Everything else points to the fact that the OMAP ROM is intervening with the boot process, as would be the case if you installed an unsigned partition over a signed one. The problem here is, you used 1st generation KF bootmodes as opposed to 2nd generation KF bootmodes to put the device in fastboot. There's no telling what those bootmodes do to the device, but clearly it's not a good thing.
I would suggest you somehow make sure your factory cable actually works, and do everything you can to get your device to stay in fastboot.
