Need to set phone completely back to stock.

My phone has become some sort of messed up. I can't make phone calls in places I used to be able to, 4G doesn't work and after trying to set it back to stock the Nagbox went away, which I would be fine with, but it needs to be there to be fully stock. Now, here's my question, can someone please link me to and give me a list of instructions on how to get it so far back to stock that it's like I just got it from the store?
