From the fine print on the rebate form:

End-user consumer purchasers who purchase qualifying LG phones and LG or generic accessories at participating T-Mobile retailers in the following states: ME, NH, VT, MA, RI, CT, NY, NJ, DE and PA
during the Program Period of August 8 – September 7, 2014 will be eligible to receive the specified LG Visa Prepaid Card by mail-in rebate.
