[Q] Voice Call volume increase to maximum

I am using XT925, non-rooted, stock rom, 4.1.2

Sometimes I found the voice call/in call volume changes to maximum itself.
From I just leave the phone alone to talking on the phone.

I am not sure what is controlling this volume.

Any ways to check which apps or system or any sensor change it?

A BT device can set the volume from it's end.

May I know what is "A BT device"

Bluetooth? I haven't connected any device.
