Is my Galaxy S4 on its Death Bed?

I've been using Eclipse for over a month now with the KTweaker settings:
Clock Speed: 189Mhz - 2.1Ghz
-40mV for 189Mhz to 1Ghz,
-75mV for 1Ghz-1.8Ghz,
(-55mV) 1120mV for 1.9Ghz,
(-15mV) 1160mV for 2.0Ghz,
(+15mV) 1190mV for 2.05Ghz,
(+35mV) 1210mV for 2.1Ghz.
GPU: Stock Voltage, 600MHz.
I've also made a hardware modification a few weeks ago. I noticed that the phone would throttle when doing benchmarks. I tore the phone down and replaced the CPU thermal pad with thermal paste. The phone no longer throttles. And because I was worried that this would cause a higher temp for surrounding components since the heatsink would be allowed to reach a higher temp. I've lowered the thermal throttle. In KTweaker Thermal, I've lowered the Control Temp Limit Start from 70 to 67 and the Temp Limit Stop from 67 to 63.
I've been using these same settings without changes for about 2 weeks or so without problems. Until last night.
Yesterday around 7PM the phone started suddenly having issues. It was on my desk and received a message, the screen lit up, I went to read the message, but I was to slow and the phone did the CRT Off animation. Immediately after the animation finished I hit the home button, but it just gave a slight vibration. It was completely lifeless for about 30 seconds, then it vibrated, waited 10 more seconds, then vibrated again and displayed the "Samsung Galaxy S4" boot screen. It booted fine. However, I haven't seen the Galaxy S4 boot screen since it's been rooted, normally it displays "Samsung Custom", so that was out of the ordinary. Once booted, I still had Root.
Between 7PM and 9PM the phone did this 2 more times. And each time, It always happened after re-waking the phone directly after the CRT Off animation. After the third random reboot I went into KTweaker and put everything back to stock then rebooted. I was going out and didn't want to have any more problems, just a working phone.
Around 10PM, in the middle of typing a text message, the phone just froze. I had to do a battery pull.
Half an hour later, the phone received a call. The vibration notification for calls is, "BUZZZZZ Buz Buz, BUZZZZZ Buz Buz" - that's how I knew it was a call. During the second iteration it just froze in the middle of a vibration and then just kept vibrating and not responding to any input. This happened right before the point the screen usually turns on to show who's calling. After a minute of vibrating it finally stopped and the screen finally turned on, but with the greeting "Samsung Galaxy S4" boot screen. This time after it booted, Android said it was in "Safe Mode." Also non of my contacts were showing, all calls and texts were referenced by the persons number and not their name.
I rebooted it again to get out of Safe Mode, after it booted, I noticed that all my contacts no longer had display pictures. It froze and rebooted 2 more times last night. Each time it seems to happen at the point when the screen is supposed to turn on, except for the time it froze during texting.
Last night it was able to successfully run, "Sleep As Android" sleep tracking without restarting and seemed to be working fine. It's worked fine all day until a little bit ago. I was browsing on chrome and the screen shut off due to the 1 minutes display timer. I went to wake it back up with the home button, but it just sat there again, and again after about a minute it was rebooting.
In the last 24 hours, its crashed and rebooted 8 times, once going into safe mode. Prior to this, the phone had 3+ days of uptime.
Is the phone suddenly dieing? What should I do?
