I am on stock 4.1.2 ROM with latest updates (MC3). I rooted my phone using chainfire method and did triangle away but still i am having system status as modified. When i check flash counter, its coming as 1 everytime even though I have stock recovery

Found a way to resolve it. After rooting, do a triangle away and then wipe your data (Factory Reset) and done. No problem after that. Just remember, Do not use triangle away again, because on Note 2 it flashes a custom kernel before it shows you the status which resets the status again to Modified.
After you do triangle away and factory reset, go to download mode and check. Everything will be in official status.

This works perfect for me now.
Device status still showed modified/custom because device was still rooted, even though having stock rom.
When current and custom binary are fine (no and samsung official), then just goto settings, save and restore, reset device.
This will delete all data on the device. When you're done, it will say normal in settings.
