That's the carrier logo. You can change that with a different theme or one found in the forums.

I don't have a theme applied. I just flashed the current 1.9.2 update and it was there...

Its there by default, I think you can take it way in settings.

Is this EXT4? I have flashed a couple times after reverting back to ext3 and when I flash the system is formatted to ext4. I searched the thread with no luck. Thanks

It should be ext4. However the issue that your explaining is something known with CWR
You might wanna try the 4ext recovery. Since I have converted to all ext4 flashing a rom has never reverted me to ext3
