And what about radio version?
Or try the new ICS leak. It's really good (for a leak).
How to check the radio version?

I have never had the phone on for more than 24 hours without pulling the battery.

Settings -> About phone Software information -> More -> Baseband ver.
There's no place like 127.0.0.1
History -> FSC Loox N560 -> HTC Diamond 2 -> ZTE Skate -> HTC EVO 3D -> Samsung Galaxy SIII + Galaxy Tab 10.1

I forgot that I flashed new baseband, so I don't know which ver. I had before. But as I said before. This is not normal.
Only recommendation I can give you, is to flash the ROM again, without any app, and try if the problem occurs again. If not, try to install all apps one by one, and maybe you find the problem (it must be an app which makes all this problems).

Have you flashed the radio firmware that came with the leaked ICS?

Just tried it myself: same here, ROM does not recognize card (32GB sdhc).

Btw, one can convert a FAT SD to NTFS on PC by typing:

Code:
convert <drive letter>: /fs:ntfs /v
No need to format and copy again (works only one-way, though)

Cheers!

Had some trouble applying the new Radio, it flashed fine, but hung in boot animation (flashed twice, two sources, no difference).

Did nandroid restore and flashed radio by only keeping radio.img, android-info.txt and rcdata.img in the zip file. Worked flawless, and leaves the splashscreen intact (should have thought earlier about this )

Cheers!

@Lee, will you be implementing this in the upcoming ICS ROM? It seems to give a welcome boost to the GPU performance
