Also happens if you are messing with over clocking and live oc and it causes your phone to freeze and reboot. Or it freezes and you have to do a battery pull.

Wipe battery status in recovery > advanced menu AFTER charging phone to 100% while powered off.

This is redundant. every time ics charges to 100 the stats are wiped.

I've experienced the phantom charge also. I fly a couple times a week and notice it mostly after I turn phone on. For example, turn phone off before flight at %50 battery. 3 hours later when I land I turn it on and am at %27 battery. If I leave it alone, it will slowly start creeping back up and within an hour or so will be %40 or higher.
The flying has nothing to do with it, just using it as an example.

Found an easy way to apply status bar themes. I use ES file explorer, browse to miui/themes and copy the theme you want to another location. I use /tmp. Then, change it from mtz to zip and extract. Go into the theme folder and copy com.android.systemui to data/system/theme. Do a hot reboot and you should have a themed status bar.

A bit more trouble than theme manager, but its working fine for me.
