The proximity sensor's red light is always lit. Please fix it as it causes major battery drain.

Do you run a notifier app? (e.g. Notifier Pro Plus or iPhone Notifications)
If so, open the app -> Default Settings -> General Settings -> Proximity sensor. Uncheck it.
If not, factory reset -> format /system -> reflash latest nightly.

If you don't use Exchange service then go into Manage Apps, find Exchange and disable it.

Mediaserver is the process that scans your USB storage for music, videos, pictures, etc... when you have a bad file or a file it doesn't recognize it sometimes goes into an infinite loop and uses 100% of your cpu load, you can either kill the process or try to figure out what's causing it, the second option may take a while but should fix the problem on a more permanent basis.

I think this is the problem.

The sound at 13:35:12.290 get set correctly to volume 12

Code:
04-15 13:35:12.290 E/AudioHardwareYamaha( 1864): setHeadphoneVolume: Set Headset Analog Volume control(12)
04-15 13:35:12.290 V/yamaha::media::CManager( 1864): setHeadphoneVolume(volume=12)
04-15 13:35:12.290 V/yamaha::media::VolumeCtrl( 1864): VolumeCtrl::setAnalogVolume(id=1, volume=15)

Then at 13:35:12.315 gets set to Volume 1
