If you know where the fastboot program is on the computer, right click my computer, go to advanced settings(I think), click the environment variables button, scroll down to the paths line(if you have android-sdk installed it will have that path in it), and make sure the path to where fastboot is is there, or copy fastboot to one of the paths it already has.
All of that is from memory, I'm on my mac right now, but I think that will get you there- google environment variables if you can't find it..

Pretty sure you didn't have to relock bootloader the first time if you were s-off, or maybe there is a slight difference between the way it flashes different operating systems(2.3 versus 4.0+) versus upgrades of the same os..?

I did not relock the bootloader either when I installed the first one. Will try the same again after I back-up.
[EDIT] Just flashed this same as before (S-Off): renamed to PH85IMG, placed in root folder, rebooted, accepted update, flashed recovery and SU, and that's it. Btw, as a Telus user, I have to say reception sucks.
