Why not just go whole hog and write a ROM yourself? Sounds like you love a challenge lol turning off fastboot only means your phone will actually power completely off when you want it to, all else works the same
This happened to me too on GB MeanRom after installing Leedroid. Sorry to see it's still an issue.

I have Leedroid installed but didn't change the settings for the capacitive buttons and mine turn off completely when the screen goes off. Must be a problem with Leedroid.
Okay, I need to ask the direct question. I looked at some videos of Sense 3.6 with ICS on obviously phones other than the 3D. They all seem to have the four icons Phone, email, messaging, camera on the lock screen and also, once unlocked, on the navigation bar at the bottom on either side of the All Apps button. These icons are not there on my lock screen or navigation bar once unlocked. How can I add them, or is it just not there on this version of ICS?
I appreciate any help or insight.

Whichever apps you add to the bottom bar (long press and drag and drop), those will appear on your lock screen.

You have to add the apps/shortcut/widget you want on the navigation bar. I long pressed on a blank space on a panel. Selected the app/widget/shortcut on to the panel. Then I long pressed on the new app/widget/shortcut and dragged it to the navigation bar. There is a setting to get the navigation bar icons to show on the lock screen under personalize/display. Also you can put as many as 4 in 1 space.

I think you might have that backwards since you can't get into the bootloader on the phone and enter the "fastboot usb" mode (using VolumeDown+Power) unless you turn off the Fast boot option in Settings/Power or take the battery out to reset it.

Lock screen icons are now determined by the shortcuts you have in your launcher bar (to the left and right of the "open application drawer" button). If you don't have any icons there (or if you've uninstalled Rosie), you won't see any icons on your lock screen.
