As soon as you finished downloading above SD card formatter file, Go ahead and unzip the folder. You will see an EXE file named "Setup". Double clicking on it will open up a setup and will install a program on your computer named "SDFormatter"
Open the above program and select the appropriate windows drive you assigned to your SD card (in my case it was H:\)
DO NOT change any option; just click on format. ignore any warning message (s) that shows up.
Now, once you finished formatting your SD card go ahead and download this file, This is a Windows Image Burner utility to properly burn BN Factory Image on your SD card: https://launchpad.net/win32-image-wr...ger-binary.zip
Step 2:
Unzip the "BN_Recovery_1.42" File you just downloaded on your desktop.
When you unzip the file, you will see a folder named "bn_142_factory_recovery" on your desktop
Under that folder you will see an IMG file names "repart"
After unzipping you will see bunch of files. we are only interested in "Win32diskimager". This is an EXE file so go ahead and double-click on it
You will see a blank image path, a folder icon next to it followed by a drive path (defaulted to the drive you assign for your SD card)
Click on that blue folder icon and locate the IMG file named "repart" under BN_142_factory recovery folder on your desktop (or wherever you saved it)
once you select the image path to correct file, go ahead and select the correct drive for your SD card (in my case it was H:/)
once this has been done, click on "Write". You will have a warning message pops up saying this process will completely erase your SD card. JUST go ahead and click "ok"
Now, sit back and enjoy. In 1-5 minutes you will see "DONE" dispalyed on lower left hand corner of the program windows "Win32diskimager"
Once you see that "DONE" displayed - remove your SD card from your computer
Put that SD card in your Nook Tablet
Connect your Nook to your PC/Laptop
Hold the power button of your Nook until you see "n" logo pops up
release the powe button, in 1-2 minutes you will see a messsge "Please DO NOT turn off your device.....installing new software"
Now DONT do anythign to your Nook - Just sit back and relax
Once Nook finishes installing IMG, a "Green Checkmark" will appear on top right hand corner
Remove your SD card from your Nook Tablet
You will see Nook will restart itself
Then you will be see a message displaying "Clearing Data"
Afterwords you will see a message displaying "Installing factory Software...Do not Turn off your device"
Once the above process finishes, your Nook will restart itself and then you will be see a "regular Nook setup" screen
Now, you have yourself a Complete Factory Version of Nook; Please Note that THIS will WIPE everything. That means you will have a Brand New Nook just like when you first bought it from BN. A clean and EMPTY NOOK!
This will erase ROOT, Your Personal Information and Data.

Update; If you have followed every steps from above instruction and still getting "please contact nook support" then you have "erased" factory partition table which means that now your device doesn't have any "build history" associated with it, like serial number, build info and so on. That means that factory ROM will not be able to run without getting that information from your device so your best choice is to call BN or go to BN and get a warranty or replacement service.

You don't really need to sdformat the sdcard before since the image contains a partition table with 2 partitions already, however you will need that sdformat tool to clear the 2 partitions and reuse the card.

after removing the sd card and let the tablet reboot i get a error screen saying "please restart your device and try again if you encounter the same issue, please contact customer service..." tried the whole process three times and it's always the same thing please help

Umm.. Weird.. Did you finished completely? I meant to say that did you see a green checkmark on top right corner? And then you tried to remove the card?
I did this many times on different nook tablets and it worked every time. Please report back in more detail so that I can try to replicate same issue at my end in order to help you out.
thanks for this man I really apreciate it! it appears that the error was the recovery, i flashed cwm on internal and got a recovery bootloop, my tablet was only entering in cmw recovery, the solution was to flash the stock recovery and then everything went just fine no errors and get my NT stock again, thank you again for this excellent tool!
I got to this step and instead of getting the green checkmark, a screen came up saying system was going to shutdown and be sure to remove sd card.
Where do I go from here?

It is just boot looping at the n screen
You did not say if you have an 8/16GB (good form) but it should not matter.
Try the entire process again. If if repeats the error, you may have gotten a bad download. Try getting a fresh copy and rebuild the SDCard.

I have tried twice with two different downloads but will try again. Sorry, I have a 16GB. I am currently trying to restore the internal partitions but will re-download and try to build another sdcard and try again.
